AccessHealth clients will have access and be directed to free or substantially reduced prescription medications through Welvista and other prescription assistance programs.
Yes. AccessHealth clients can visit you at your office. We will arrange transportation for patients and will provide ongoing care navigation and case management services as needed.
While AccessHealth nurse's provide an initial assessment of a client's needs, AccessHealth is not a medical clinic. Instead, we connect uninsured clients to a medical home at one of more than 100 primary healthcare providers, including those at St. Luke's Free Medical Clinic and the Middle Tyger Community Center Clinic. AccessHealth also connects each client to community resources and programs designed to improve their health and overall quality of life. We provide care navigation and complex case management, and our community health workers are trained to identify each client's barriers to care and then create a plan for overcoming them.
More than 23,000 adults in our three-county service area have no health insurance of any kind, nor Medicaid or Medicare. These people are our friends and neighbors, and many of them work more than one job to support themselves and their families.
