Connecting People to Resources

Community partnerships help clients access assistance and build healthier lives

AccessHealth's care navigators aren't just knowledgeable about the local health care landscape. They are perhaps the most comprehensive experts on the full range of social services, programs and resources in the community. Collectively, the team at AccessHealth has built a network of partnerships and trusted relationships with dozens of organizations in Spartanburg, Cherokee, & Union counties that provide assistance to people in need.

From housing assistance and SNAP (food stamps) to job/training programs and legal assistance, AccessHealth care navigators know not only what is available, but the rules, regulations, requirements and procedures for each program. After completing a needs assessment, care navigators spend time getting to know each client and helps direct them to the programs and assistance that they need. This includes:

  • Emergency Housing
  • Rent Assistance
  • SNAP
  • Food Pantries
  • Legal Aid
  • Behavioral Health Resources
  • Documents Assistance (Social Security cards, birth certificates, etc.)
  • Unemployment Assistance

In many of these cases, there are multiple options and programs. Our care navigators have a deep understanding of how each are different and which will best serve each client's unique needs. This ensures each person gets the help they need more efficiently, while increasing their ability to manage their health and improve their lives.
