Office Visit Procedures

We look forward to seeing you at our new office located at 501 Howard St., Suite B. Parking for our new location is off Raindrop Street behind the building. The main entry faces the parking lot.

Upon entering the office:

• You will be asked Covid-19 screening questions
• Your temperature will be taken
• Masks are required for the duration of your visit
• All guests are asked to practice social distancing
