Martha Alvarado joined AccessHealth Spartanburg in 2016 as a Bilingual Care Navigator for the Healthy Outcomes Program Spartanburg (HOPS).
Martha is responsible for proving direct client care by case managing, navigation and coordination services. Also, Martha conducts medical, social and medication assessment in order to create an individual care plan for a client. In addition, as a care navigator, Martha assists with referrals by making connections to other community resources, providing education to help clients prevent chronic diseases and live a healthier lifestyle, conducting home visits and escorting clients to medical appointments. Martha sets no limitations to overcome barriers to achieve client’s needs.
Martha brings 13 years of experience working as an advocate and patient benefits counselor in a community health care clinic that served a large and diverse population. Martha is part of the Hispanic Alliance Committee and reaches out to the community once a month during St. Matthews Episcopal Church's free medical clinic day.
Martha is originally from Mexico and has lived in South Carolina for the past 23 years. She has two daughters and one son. In her free time, she enjoys spoiling her three adorable granddaughters with goodies!
